Kentucky Eye Care Alliance, Illuma Care Connections Partner to Close the Diabetic Vision Care Gap

(PRESS RELEASE) ATLANTA, GA – The Kentucky Eye Care Alliance (KECA) and Illuma Care Connections announced a partnership committed to improving eye health and lowering the risk of blindness among more than 15,000 diabetic patients throughout Kentucky.

The unique healthcare program is based on Illuma Care’s proactive patient outreach and proprietary technology platform, which makes a critical connection between leading eyecare professionals and diabetics, many who often delay vision care. By proactively scheduling at-risk diabetics with professional eyecare providers, Illuma Care Connections and the Kentucky Eye Care Alliance, including members of the Professional Eye Care Resource Cooperative (PERC), are having a significant impact on blindness and retinopathy among diabetics on Medicare and commercial health plans.

The Illuma Care Connections team thoroughly tracks the eye treatment of each diabetic patient and the overall percentage of retinopathy and other eye diseases across the patient population, through complete clinical findings and analytics that contribute to a better quality caregiver performance.
